I've experimented with: calibrating the bed, very small gap between nozzle and bed, high and low bed temperatures, high and low filament temperatures, fan on/off.

When the print head moves to the right, it prints poorly - the line is narrow (round top) and sits almost on top of the print bed. When it moves to the left, it squeezes flat into the print bed like it should for a first layer.

Does this mean I have a bad nozzle?

What you're describing is most likely not caused by a bad nozzle, but instead by a mechanical or calibration issue, most likely gantry tilt, X-axis misalignment, or uneven bed leveling across the X-axis.
